Each resident referred services will be subject to a pre-screening to identify their needs and eligibility for the program. The assessment process will be completed by the Program Director, Program Manager, and Case Manager. The pre-screening process will allow the assessor to obtain important information to assess the child’s behavior, the onset and duration of the problems, and mental status. The prescreening will also identify prior services, current and past medication, and medical history. Further, the prescreening will identify current and past substance use or abuse, including co-occurring mental health and substance abuse disorders and at-risk behaviors to self and others.
This group will be conducted by a Licensed Clinical Professional. Counseling Sessions will help educate the young men about their mental health diagnosis. The groups will also give the young men the ability to discuss and address issues and concerns regarding his disability. This group will also deal with medication compliance and medication management conducted by a licensed nurse.
The Process Group is designed to identify the needs of each individual youth. These sessions will allow youth to understand how important they are as individuals. The group will also teach the youth how to develop trusting relationships as they begin to better understand themselves and others. This group will also allow the youth to recognize positive qualities within themselves and promote peer support. Support groups will be held weekly by a Licensed Clinical Professional.
Family Counseling is designed to help rebuild the family structure. In order to reunite the family reconciliation and restoration must begin. This group will give families the opportunity to deal with past issues and look forward to a better and brighter future. The group’s ultimate goal is to assist with healing of the mind, body and soul. This group will be facilitated by a Licensed Clinical Professional.
Through case management, the Case Manager will coordinate community resources to assist with improving coordination and continuity of care. Through collaboration with the resident’s school, we will include teachers, counselors, and any other officials in the case management process in which to provide insight on the resident’s performance.
This group will be conducted by Safe Haven Transitional Youth Services staff. This group is designed to teach the young men how to develop strong communication skills. Through proper guidance, the youth will learn self-respect and leadership skills. The youth will learn how to effectively communicate emotions. The ultimate goal that the young men will learn the necessary skills to obtain employment and manage in society.
This group will be conducted by Safe Haven Transitional Youth Services staff. The main focus of this group is to teach the young men the necessary skills to live independently after program completion. These basic skills will empower the young men and prepare them for challenges that will be faced daily. They will also be taught strategies that will help promote productive lifestyles and relationships. These skills will enhance their ability to set and accomplish their goals. (Cooking, Cleaning, Money Management, Employment, Educational Skills, Budgeting etc.)
Medication/ Psycho Education sessions will be held to educate youth about medications that are administered. These sessions will be held monthly or as new orders are prescribed by physicians. Youth will be advised of the adverse side effects and effects of each medication. This session will be conducted by a Licensed Nurse/Psychiatrist with assistance from the staff of Safe Haven Transitional Youth Services.
This group will be conducted weekly by a Licensed Clinical Professional and co- facilitated by Safe Haven Transitional Youth Services staff. These sessions will teach the young men different strategies and techniques to deal with internal and external pressure. The young men will learn how to express their emotions, feelings and concerns without causing harm to themselves or others.
This group will be conducted by Safe Haven Transitional Youth Services staff and co-facilitated by Safe Haven Transitional Youth Services residents. Community meetings will be held nightly to discuss program expectations and objectives. The young men will be given the opportunity to voice their concerns and give suggestions to better enhance the program.
