Children can be referred by Department of Social Services, Department of Juvenile Justice, Court Service Units, Community Service Boards (CSB), and Private Agencies.
Listed below are some of the required documents that is needed to make a determination if the youth meets the admission criteria for placement. These documents may be faxed, mailed, or emailed to the facility.
Once the above documents have been reviewed a face to face interview will be schedule to make a decision on placement. Prior to the youth being placed, an Independent Assessment, Certification and Coordination Team (IACCT) and Certificate of Need (CON) needs to be completed.
The place that offers Children’s Residential Service for at-risk adolescents. Safe Haven Transitional Youth Services goal is to provide the best quality services to its consumers and community. The home provides care for up to five adolescents in a family-style setting. Children living in our group home will continue to attend public school. Our program is designed to meet the needs of individuals experiencing difficulties in their lives due to emotional, behavioral, and/or mental issues
